if we can not read your information.
Please note that the Code of Conduct Form, which prints off with
the Registration Form, must be sent in with the Registration Form
even if you have sent one previously.

This form is to be completed by all accepted graduate students
for all graduate programs except health education and Certificate
of Advanced Study. Please read form for specifics on eligibility.
This form is to give us permission to share your personal information
with NewYork State Education so that we may recommend you for certification.
This is not a certification application. You will need to apply
on line for certification through the New
York State Education TEACH system.
This form is to change your name of permanent address. Please note
that you may change your own local address on Banner Web.
Change of Schedule Form
The drop/add period for Spring 2008 has ended.
Students must now use the Withdraw from Course form to be removed
from a class. This form is to be completed if students wish
to drop or add a class and is only used prior to the start of a
semester or during the first five days of the term. Please send
the form to Graduate Studies, PO Box 2000, Cortland, NY 13045; or
fax it to 607-753-5988.
The Graduate Diversity Fellowship funds graduate-level study for
U.S. citizens (or permanent resident aliens) who will contribute
to the diversity of the SUNY Cortland campus. Financial assistance
from the fellowship program provides students with tuition and stipend
support. Yearly awards may vary.
Diversity Fellowships are in the form of financial support and
carry no work requirement for the recipient. Fellowships are renewable
if funding is available and if the recipient maintains a minimum
3.0 cumulative graduate grade point average. Award amounts may
vary each year. Typically, awards are announced in late spring
for the following academic year (fall-spring). If any funds remain
after the academic year awards have been allocated, applications
for spring and/or summer fellowships will be considered.
Graduate Opportunity Program awards provide tuition support to
matriculated graduate students who were former Educational Opportunity
(EOP) program, Higher Education Opportunity Program (HEOP), or
SEEK (Search for Education, Elevation, and Knowledge) program
students as undergraduates. Award amounts depend on the number
of qualified applicants and the allocation SUNY Cortland receives.
This form is to be completed by accepted graduate students, after six credit hours have been taken at SUNY Cortland and candidacy is established (if required). May request up to six credits total from other institutions.
This form is to be used only if you have previously received an acceptance letter for a degree program and are now changing programs, declaring, or changing a concentration within the program to which you are admitted.
Once you are accepted and start taking classes it is expected that
you will take courses every fall and spring term. If for some reason
you are unable to take courses, you should complete this form and
submit it to the Graduate Studies Office.
After the first five days of classes of the fall or spring semester,
or the first day of a summer or winter term, students should use
this form to withdraw from courses instead of dropping.
If you plan to completely leave the College and not return, please
complete a Withdraw from College form.
