Some of the forms found on this page are
PDF documents written in the "forms" format and can be used in one of
two ways. You can print the file on your printer, fill it
out, then submit the form in hardcopy to the Dean, much the way you
have done in the past. An effective alternative is to
download the form as a file to your computer, open and fill out the PDF
document on your computer, save it for future reference, then print it
out and submit it to the Dean. The "forms" format allows you
to fill out designated areas without modifying the overall format of
the document.
